Documents to Submit
In order to complete your application, you will need to provide the following documents. You may submit them online, email them to becca@reasonandrepublic.com, fax them to 864-555-1212, or mail them to 1650 E. Greenville St, Suite H, Anderson, SC 29621. If you need assistance, please call 866-983-5449. We need the proof of residency document and proof of student's age before the packet can be processed.
Immunization
The following documents are required for all students.
A child cannot be admitted into any school or child development program without a valid SC Certificate of Immunization or a medical, religious, or special exemption.
Immunizations required by SC law are Tetanus containing vaccines (DTaP, DT, or TD), Polio, Measles (two doses), Rubella (German Measles), and Mumps.
Hepatitis B Immunizations are required for children entering all grades.
The Varicella (Chickenpox) vaccine or history of the disease is required for all children admitted to K5 through 12th grade. Starting in the 2016-2017 school year, all K5 children will need two Varicella vaccines that are at least four weeks apart with one dose on or after the first birthday.
If your child does not have a valid certificate, take his/her shot records to your physician or the County Health Department. Should additional shots be required, a parent or adult authorized by the parent must accompany a student under 16.
As of the 2016-2017 school year, all 7th, 8th, 9th, and 10th-grade children are required to have a Tdap vaccine.

Technology Agreement

Home Language Survey (HLS)
The Civil Rights Act of 1964, Title VI, Language Minority Compliance Procedures, requires school districts and charter schools to determine the language(s) spoken in each students' home in order to identify their specific language needs. This information is essential in order for schools to provide meaningful instruction for all students as outlined in Plyer v. Doe, 457 U.S. 202 (1982).
The purpose of this survey is to determine the primary or home language of the student. This survey is given to all students enrolled in the school district/charter school. The HLS is administered once, upon initial enrollment in South Carolina, and should remain in the student's permanent record.
If a language other than English is recorded for ANY of the survey questions below, the appropriate identification screening assessment will be administered to determine whether or not the student qualifies for additional English language development support.
